Demo Ride Waiver of Liability
I acknowledge and accept that cycling is a dangerous sport that may result in personal injury, or even death. I declare that I use this Juiced Bikes test e-bike at my own risk, and further agree that I am responsible for any loss or damage to this test bike.
The undersigned hereby agrees to assume the risks of participation and releases, and forever discharges and holds harmless Juiced Bikes its officers, directors and staff, and participants, from any and all actions, liability claims and demands upon or by reason of any damage, loss, injury, or in connection with test rides, from the Juiced Bikes staff and all parties associated with the sponsorship of this course and or demonstration.
He/She hereby waives all his/her rights to the claims, actions, cause of action, demand or suit of loss, injury, damage, or suffering sustained.
I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign freely and voluntarily without inducement.
This is to certify that I, as participant, do consent and agree not only to his/her release of Juiced Bikes but also to release and indemnify from any and all liabilities incident to his/her involvement in these programs for myself, my heirs, assigns and next of kin. I acknowledge that I am 18 years of age or older, will wear a helmet, and obey the local bike laws.
If the demo rider is under 18, this waiver must be signed by that rider's parent or guardian. Parent/guardian ensures minor rider will wear a helmet and obey California e-bike laws, including prohibiting riders under 16 from operating a Class 3 e-bike.
California e-bike laws prohibit riders under the age of 16 from operating a Class 3 e-bike. Juiced Bikes strictly enforces this regulation and will only allow riders under 16 the option of demo riding a Class 2 e-bike.
By signing this form, I grant Juiced Bikes the rights to use my likeness and voice recording in video and photography without payment or other consideration in relation to the promotion of the Juiced Bikes company and branded products. I waive the right to approve the finished product wherein my likeness appears and understand my likeness may be used in some of the following manners: Online / internet videos, Online ads, Social media channels, JuicedBikes.com
Juiced reserves the right to refuse or suspend any demo rides at its sole discretion to protect the safety of the e-bike rider.
